documents required when women applying for passport for first time after marriage

Dear Sir/Madam,
Kindly tell me the importance of ANNEXURE ‘D’ when applying for Passport.
I'm applying passport for my mother(Age:52) then agent asking me to submit form ANNEXURE ‘D’.
All my mother certificates/BankAccounts/AdharCard/VoterId etc.. having surname as her father surname (before Marriage) only.
Could you please confirm me the below 2 things..
1. If my mother submit ANNEXURE ‘D’ then the passport will come with NEW surname (my father surname/after marriage) is it correct ??
2. If my mother did't submit ANNEXURE ‘D’ then the passport will come with OLD surname(her father surname/before marriage) is it correct ??

Please help me to provide the above information.

Following marriage, remarriage or divorce:

(a) A woman applying for change of name/surname in existing passport due to marriage must furnish:

(i) Photocopy of the Husband’s passport, if any, and

(ii) An attested copy of marriage certificate issued by Registrar of Marriage OR an affidavit from the husband and wife along with a joint photograph, (Specimen at Annexure D).

(b) Divorcees applying for change of name OR for deletion of spouse’s name in existing passport must furnish

(i) Certified copy of Divorce decree.

(ii) Deed poll/sworn affidavit (ANNEXURE ‘E’)

(c) Re-married applicants applying for change of name/spouse’s name must furnish:

(i) Divorce deed/death certificate as the case may be in respect of first spouse, and

(ii) Document as at (a) above relating to second marriage.

II. In other circumstances for change of name, the applicant (both male and female) should furnish:

(i) Deed poll/sworn affidavit (ANNEXURE ‘E’)

(ii) Paper cuttings of two leading daily newspapers in original (one daily newspaper should be of the area of applicant’s permanent and present address or nearby area).

Understood from your message that everyone (married people) need to submit ANNEXURE ‘D’
is it correct ?

Could you please clarify me the below 2 things also...

If you want passport in NEW SURNAME then it is necessary to submit Anexure D.

Annexure D is the affidavit to be submitted along with the application for pass by a women applicant for change of name after marriage and this is joint affidavit along her husband. Since all her documents are in her maiden surname name, she can apply for Pass Port without Annexure D in such case she will get the pass port in her maiden surname. Practically this will not pose any problem.
