Simply write down the proceedings of the meeting, any decision taken or suggested, get the signatures of all the members present. Use the same book for all the meetings. The page numbers should be continuous. This is called the minutes.

Sir,
In addition to the information given by sri Arunagiri, you please ensure that the quorum and the decisions as per the bye laws.

There is no format as such. Minutes have to be written in such a manner that they reflect an honest account of the proceedings and the decisions taken at the meeting. It need not be a word to word account of whatever is said and countered at the meeting. It may be a prcise account of the proceedings. When there is a datailed discussion, it is enough to write-"The General Body has after detailed discussion decided or passed the following resolution...." Take care to mention the proposer and seconder of each resolution passed at the meeting. If there is a division in the voting meneion the number of votes polled for and against the proposal. If the resolution is passed without any division, write that it has been passed unanimously. This is very important as otherwise the resolution may itself become questionable/ contraversial. The other precautions mentioned by Mr. Arungiri also may be kept in mind.
