Verma ji! The fee DD/postal order should favour Assistant State/Central Information Officer-cum-concerned police officer (from where you want to obtain the required information).

Format of application, mode and amount of filing fee under RTI is decided state-wise as per RTI rules of that state. Since police is a state matter, you should follow RTI rules of the state, where police station is situated. I have posted details of some states in jps50 files. For all states please visit www.rti.gov.in.

Accounts officer is not the proper person to supply the information in police stations so move another application.

apply to the PIO (Public Information Officer) of the concerned police station.

depending on the state,there are different format for the same as quoted by Mr.JP Shah
