Conversion of religion procedure from hinduism to islam

Dear Experts,

My Name is Zaheer Khan & My Wife is Sonal Sharma.

My wife and I got married in 2012 through special marriage act at the time of marriage my Wife was a Hindu by birth and I am a Muslim by birth after these years my Wife has experienced the Religion of Islam and she on her own will and power wants to embrace Islam and would like to convert her Name and Religion legally now.

At the time of Marriage her name was a Hindu and her Religion was a Muslim.

She wants herself to be a revert legally through Gazette certification please help us to understand to process completely we have gone on the Website of Maharashtra Government we found the documents required and the process also but have some doubts which needs an experts clarification. So we request you to please guide us and help us to complete this successfully.

1. Can we process the Change of Religion and Change of Name forms at the same time or do we have to first receive the Gazette for Change of Religion first and then later on apply for Change of Name or the other way around.
2. In the Change of Name Form & in the Change of Religion Form we have been asked to mention Reason for Change according to my Wife she does not want it to be Marriage as according to her it is not because of the Marriage she is willing to Change her name and Embrace Islam but it is because of the Faith in Islam and she wishes to have this Reason to be mentioned in the Gazette hence we need your expertise to go ahead in this matter.
3. Once the Gazette for Change of Name is completed then can we make the changes in the Marriage Certificate which was received by the Court through Special Marriage Act or even this is required to do is for you to confirm us.

We have the affidavits ready through some of our family friend but we want your expertise to understand these matters with more clarity and views from your support.
