Change of child surname

Dear Team,

I have changed my name after the birth of my daughter. Name was changes by following appropriate procedures in Gazette.

Now In my daughter's birth certificate , my old name is mentioned. While in school and other documents, my new name is updated as last/surname.

Now my daughter wants to apply for passport.

Should I change my daughter's name(Last Name) also through gazette ?

Or

Can she submit my change of name document as proof ?

Please advise.

Since your daughter's name has not been changed and it appears the same what is in the educational records, an affidavit confirming the surname added later after registration of birth in addition to a copy of the document confirming your changed name shall be accompanied to the passport application giving details, this will suffice the formalities to be complied, send it, wait for the reply, if it is okay, go ahead or you will be guided with mor3e formalities by the passport authorities and you can follow them accordingly.
