mothers name printed wrong

Sir/Madam

I have finished my 10th in the year 2008 from CBSE board. My mothers real name is Kanta Kanwar, but in the memo is Shanta Kumari and ever since I have used that name i.e, in my 12th and B.tech.
Now i want to correct my mistake and change mothers name to Kanta Kanwar in the memos

Please help me through this process.

There is standard procedure stipulated by CBSE wherein you need to submit an application for rectification alongwith the proof of correct name duly gazetted
