Passport query urgent pls help

Me and my sis both are twins,
DOB 1.3.1991
kuch saalo baad meri behen ko mere chacha ne adopt kar lia...
Ab mujhe passport ke liye apply karna hai magar mere pass koi birth certificate nahi hai...
ek birth certificate hai jo same mere birth date ka hai par uspar koi aur naam likha hai magar parents ke naam pe mere parents ka naam hai..
I'm not sure wo mere hai ya meri twin ka..
Mujhe ye bhi pata nahi ki mera birth certificate issue hua hai bhi ya nahi...
What should I do now?

Me and my Sis both are twins,
We both born on 1.3.1991
but after couple of years my uncle adopted my sister.
Now I want to apply for Passport but I don't have any Birth Certificate, and I don't know that either there is any birth certificate issued on my name or not..
But I have a birth certificate which was issued on the same date of my birth containing my parents name..
I'm not sure is it mine or my sis certificate as the certificate contain a name which was different from my name..
My sis got his new birth certificate few months ago..
What should I do now?

Contact Municipal Corporation office to issue birth certificate. In case of any discrepancy, ask them to guide about the way of rectification.

If your sister got the birth certificate, your job is very easy. Follow the same procedure and get it for you.

Now my sis is cousin to me, his maternal uncles did it for her..
Here is no one for me who can back me and do the legal proceedings...
If I make an affidavit that the certificate which I'm having now is mine and the name mentioned in the certificate is the same person I'm..
and then go for rectification of the birth certificate??

What about your school and other cerificates.How your sister could become your cousin merely because of adoption.First tell me what is your intention

My uncle legally adopted her, by the way of court proceedings...
In my all academics up to graduation and above my name is Aditi..

Basing on the certificate what you are holding meet the concerned issuance authority for mentioning of names.There should be some School Certificates also carrying the Date of Birth.With all that meet the PRO in passport office they would guide you with practical solutions.

Go and approach the same registrar office. Produce your previous birth certificate (which shows your parents name alone) and other proof which shows your parents name.
They will give a another birth certificate, incorporating your name. You can also mention that your sister got the revised birth certificate recently.
Follow the above steps. In case of problem come back to us.

Annexure E has nothing to do your present query.
You have to contact a consultant, who can get you the passport. Or the PRO of the passport office will help you if you go in person. You contact the Passport help line.
After exhausting the above, if you have any doubts come back to us.

TAKE IT CATEGORICALLY:
1)If your Date of Birth recorded in High school certificate falls before 26Jan1986
you do not need any other thing to prove your DOB before PSKs.
2)In case your DOB recorded in High School Certificate is a date after 26 Jan 1986 only a certificate issued by concerned Municipality or Registrar of Birth and Death would be acceptable.
3)Affidavit of Annexure E is for persons who are known by two names.In case the date of birth recorded in HSC is same as in Birth certificate you can use Annexure E affidavit otherwise not.
4)In case of mismatch between two you would need a FRESH DOB ISSUED BY CONCERNED MUNICIPALITY OR BY REGISTRAR OF DEATH AND BIRTH.
5)BUT YOU HERE STATE A PROBLEM,YOU DO NOT HAVE A RELATIVE TO CONFIRM YOUR BIRTH DATE,HENCE YOU CAN USE ANY AGED PERSON POSING FRIEND OF FAMILY.
HOPE YOU DO NOT NEED A RETURN NOW HERE TO PEOPLE WHO DO NOT KNOW SOLUTIONS OF A PROBLEM.

ONLY FOR CHANGE OF NAME (even for a small spelling mistake also),ONE REQUIRES TO submit paper ad. and affidavit.otherwise ssc/hsc/muncipal office crtificate/registrar of birth/death cert.is acceptable for passport issue/renewal.
passport seva kendra's pro is helpful to clarify.
i see many private persons also outside passport issue offices,who know procedures.but unfortunately law does not allow them to officially help people.
