passport name chage

can any one please asnwer my question.

my name on birth certificate was pavankumar. which i changed to pavan via gazette in september 2009 and i gave a name chage advertisement in newspaper in january 2010, now for passport i also have to submit a affidavit of name chage so how do i do so as i have already changed my name via gazette... also what should be the date of stamp paper, please reply as sson poosible.....

SPECIMEN AFFIDAVIT FOR CHANGE IN NAME/DEED POLL/SWORN AFFIDAVIT

(On non-judicial stamp paper)

(This deed poll/affidavit may be signed and attested in presence of a Magistrate/Notary or Consular Officer in an Indian Mission abroad)

Note: In case of change of name, applicant should insert advertisements in two reputed newspapers (one local newspaper of the area in which he/she is residing and 2nd in newspaper of the area of permanent address) and submit original newspapers at time of applying for passport in his/her new name.

In your case an affidavit with a copy of gazzette and the newspaper clipping of Jan 2010 is sufficient. Further the stamp paper can be current dated there is no necessity of back dated stamp paper.

Get the Gazzette copy certified by a Gazzetted officer as an abundant caution.

Make necessary changes where ever required.
