Change name altogether or a correction ?
You will need to visit your School and the Relevant Board. First understand the process which will require you to get Form from Board. Get it endorsed from the Principal with the seal of the Institution. Go back again to the board and pay the requisite fees and collect the Duplicate Marks sheet with the necessary correction on the date which board asks you to come and collect.

The school admin can provide with requisite forms, certified copies, and guide you to nearest/regional office of Board.
