may please to provide profarma of d.v.c (domestice violence act) application profarma for filing the same before j.f.c.m court under section 12 of d.v. act 2005 or section 23 of d.v. act for interim prayer.

It is better to go through the Bare Act which includes WOMEN's PROTECTION FROM DOMESTIC VIOLENCE RULES 2006,

There is no specific requirement under this Act rather the relief should come within the purview of section 18 to 22 and 23 for interim maintenance. It depends upon your facts which of the sections are relevant for your case.
