Regarding birth certificates.

i would like to know the child was born at home in thane in 2005 and his birth certificate is still pending for withdraw but there is NO DISCHARGE CARD or hospital receipt to withdraw the certificate from municipality and how they can make or apply for birth certificate in this case without required or important documents.please help

write an application to the SDM of the area with the proof of the birth of the child enclosed with a duly sworn affidavit and get the permission to incorporate the name in the birth and death register of the area. Penalty is also required to be paid as decided under relevant rules.
