The Passport officer would guide how to proceed in such case ? Almost all offices have their procedure as per their satisfaction, instructions and experience for such practical problems.

I think you can use Annexure E for the purpose, however, you may contact the passport office if Annexure E affidavit will suffice the requirement to solve the subject issue or to make a general affidavit for change of name.
