Regarding issue in name in birth certificate ,10th marksheet

Querist : Anonymous (Querist) 05 July 2017 This query is : Resolved 
Sir , the name on my birth certificate is stated as Ashish Kumar and on 10th .Marksheet it is Shukla Ashish Kumar Ashwini Kumar.and the mistake repeats in 12th as Shukla Ashish Ashwini...Please provide a guidance sir...For solving this issue
