Change in first name of individual

I was earlier called Farooq Abdultayeb Dadla and like wise my wife Farida was called Farida Farooq Dadla and my son too was called Murtaza Farooq Dadla

Now that I have changed my name from Farooq to Mustafa with other particulars remaining the same i.e Mustafa Abdultayeb Dadla

For the purpose of carrying out the change in all passport, is affidavit for change in name for me enough to carry out change in all three passports or do i need each affidavit for each one of us.

Affidavits with a copy of the change as published in the official gazzette needs to be attached separately with each application viz. your self, your wife's and son's passport.

Yes. before making affidavit, you shall apply for publising the change of name in the official gazette of your state government. You have to apply seperately for yourself, your wife and your children. You shall procure the copies of the said gazette from the gazette office produce teh same along with the affidavit as required. Every member of the family shall made seperate affidavits.
Once you have authentic copy of gazette, you may not be asked to produce affidavit in every case. That you may please confirm with the appropriate authority, since different department have different rules and procedures.

But, the change of name can be done by publising in the gazette of respective state governments too. Different states have different types of form and different fee structure.

I am from Mumbai and have with me the notification in the gazette published in Maharashtra Government Gazette.

All I need to know is whether the affidavit has to be prepared for me or for each one of us to support our request for name change in each one of our passport

You have to make affidavit of all the members of your family seperately. Your affidvait will be applicable to you only and not to your family members.
Affidavit is a declaration made on oath and verified by competent authority.
Since, the case is regarding passports of each members of the family, every members shall make seperate affidavits to effect the change of name in the passport

Hope you have published the change of name of your wife and child too in the official gazette. Then attach copy of the same along with each affidavit to produce before the passport authority.

Your change of name should also be got inserted in Gazette of State so it is better to engage any lawyer as already advised.

Dear Mustafa,

The lowest stamp duty applicable in Maharashtra is Rs.100/-. So you have to make affidavits in the stamp papers of Rs.100/- each.
