Affidavit for not having a ration card

I want to take a Gas connection from HP.
The dealer has asked me to submit a notarized affidavit for not having a ration card. Is it on a stamp paer or what.Fees etc.
Kindly tell me the procedure to get it.

Lawyer is demanding 150/- and I have heard that in court it will cost you 150-300 on your own.
