Passport change in name requirement to be fulfilled

Sir How can I get my name changed in my Passport issued in India ?
Can I get my name changed in my passport abroad also where I am residing presently ?
Secondly can I get an affidavit prepared and signed by Notary Public from any place in India other than from place of my residence ?

Why you want to change name, if it is error of passport office, approach them for rectification?

What is name in your birth certificate and other Id. documents?

You can submit application with sworn annexure E, two news paper ads of your area and other related documents.

Decision of passport office would be final.
