Notarised affidavit

Is it possible to get notarised affidfavit for LITERATE person aged 86 for date of birth proof, as he lost all school documents?

Will passport officer issue passport based on this document for age proof?

Kindly advise course of action to get passport for 86 year old literate.

What about other documents like Election Id,Ration card ,Pan card,Driving License etc.Meet the Pass Port officer in person and seek the remedy.

Birth certificate from local authority to be received if record exist.

Duplicate copy from education institute from where education received should be obtained and submit.

Arrange birth certificate from the place of birth i.e, municipality or school records. Where and what for required for such a senior citizen?

Mostly notarised affidavit only shall not be regarded as proof for DOB you will have to show some other documents like election card, pan card passport authorities ask for DOB certificate only

How one can lost all school documents. At least photo copies can be traced from some source.

As the person is senior citizen applying for passport, he must be having some bank accounts for which PAN Card might have been submitted to them. Take out the copy of that pan card which is proof of age and obtain duplicate from IT Dept if the original is missing.

Notarized affidavit from literates is not generally acceptable to Pass Port Authorities. However, in that affidavit, he can mention about how he lost the certificates, and he can submit some supporting proof like Pan Card etc., to convince the authorities.

Rely on any other documents say PAN card, drv licence, adhar card, election card, any regd sale deed;
Consult Agents who will manage it.

First either approach Passport office authority or agent , they will give correct information

An affidavit in the format of annexure 'A' attached to the passport application will be applicable only for purely illiterate persons hence in lieu of birth certificate or any other standard birth proof, alternate proofs namely Aadhar card, EPIC, PAN Card etc. can be produced and if required, an affidavit informing the loss of school certificates may be attached to the application. Consult the passport office staff for clarification.

1. Notarised affidavit may not be accepted by the Passport office as a proof of DOB,

2. Arrange for duplicate copy of birth certificate &/or school leaving certificate,

3. You can also use PAN Card, Adhar Card etc as proof of his date of birth,

4. It will be prudent to visit the Passport office to further enquire about the matter.

Follow the guidelines of passport office instead of working on assumptions and presumptions.
