Mother's name change

I am from Hyderabad. My wifes name is written as Runa in my son's birth certificate as we call her with that name in my house but in all other documents (voter ID, School certificates, Aadhar card) it is as Madhumala Bhaskar. so i want to change her name from Runa to Madhumala Bhaskar in birth certificate. Year of birth is 2012. so please suggest what should i do and the procedures to be followed.

File an application with affidavit to the effect before municipal authorities for inserting her name as Runa @ Madhumala.
If they do not agree file a suit for declaration through a local lawyer.
