Annexure d is enough for updating change of surname in passport after marriage

Dear Sir,

my wife is already having a passport with her maiden name. Now we need to get her passport updated with my surname. Kindly advice

we took Joint marriage affidavit two years back in the format of Annexure D, now we are planning to produce the same affidavit to passport authorities. is this sufficient for passport authorities or shall i get a new affidavit?

you need registration of marriage certificate for changing your wife surname in passport

currently in my wife passport there is no surname. we are planning to get my surname added to her passport.

we have the following documents to produce to passport authorities:
1. Marriage certificate issued by Waqf board as we are muslims. (having my wife's maiden name in that)
2. Annexure D.

my doubt is my wife's maiden name is mentioned in marriage certificate issued by waqf board. This will be sufficient or we need annexure D as well.

Kindly advice!
