If an marries a lady with his wife's permission, is the second marriage valid?

No it is a legally void marriage.

Section 5(i) of Hindu Marriage Act : A marriage may be solemnized between any two Hindus, if neither party has a spouse living at the time of the marriage.
Therefore the second marriage by a male Hindu is null and void even he get permission of his wife.
