What to fill in attestation form

i was native of uttar pradesh and so had my caste certificate made there, than me and my full family moved out to nearby district in madhya pradesh permanantly, my relatives stay at old up address .
now i have been selected for a central govt job i filled permanant address as my old address of up where now there letter to appear has come,relatives told me about it and i got it, now have to fill attestation form so what address i should now write in the column of HOME ADDRESS IN FULL
what will be procedure of police verification as now i live in mp not there, where should i mention my mp address , should i mention up add or discard it, what i should tell if i am asked for it.

Attestation form contains the provision of both the permanent as well as present postal address for correspondence. You will have to fill both the addresses, as police verification will be made at both the places, of permanent address as well as the present address where you live.

thanks for all your advice,
dhigra ji it contains just one home address,
even if it will contain both permanant and present and than if i put old up addrss in permanant where i dont reside anymore will it not be a false information as i have moved from there permanantly to new one?

these addresses are only meant to ensure that there is no police record in ilaqa thana

If the attestation form does not contain provision for permanent address, the form might have been printed with defect based on some official's own limited vision.
However, if there is a provision for one address give only the correspondence address where you can ensure receiving the correspondence in time and also from where police can get the needed information for verification purposes.
