i have lost my ration card. I want a new ration card. What is the procedure to get it ? And what documents are needed ?

Visit the office of District supply officer.an application and an affidavit would be the requirement.

Visit Ration Card Office, apply on prescribed application along with requisite nominal fees if any. This differ State to state.

Lodge a police GD and then go to ration card office (food supply office in Haryana) and then apply it.
