Its a minor mistake in my mother's name. Instead of Ruqaiya it is printed as Rukaiya. Q is replaced by k in the name.

It is minor mistake and passport office may avoid it. You can mention both name as Ruqaiya alias Rukaiya. However if you do so, both name would appear on the passport.

Mother's Surname is also not mentioned on my birth certificate. She is a divorcee. Only her first name is there.

You meet the registrar of birth and deaths regarding spell mistake in the date of birth certificate, he may insist for an affidavit. In my opinion affidavit is more than sufficient.
bilalahmed (Querist) 07 May 2015
Will affidavit solve my problem?? Also if mother's surname is not there on my birth certificate will it create problems if i issue passport?? Because my mom is a divorcee.

According to my knowledge, mother's name will not appear on marks sheets. If the error is in the birth certificate and HSC certificate, since it is minor error, you can continue with that error in future. Graduation and PG marks sheets do not contain mother or father's name.

malipeddi jaggarao Here in Maharashtra-Mumbai both mother's and father's names appear in the marksheets issued by Mumbai university and Maharashtra State board

This seems to be a minor typo error which either can be ignored or approach the birth and deaths registrar's office with an application to correct the same in the birth certificate by complying with the necessary formalities and get a new birth certificate with corrected name. If the surname is not there, what are you worried about, let it not be there.

Contact passport officer for necessary action, if raised any objection.
