Change of name

Dear Experts,
I seek your advise with following facts:
My son was born in November,1989 in Chennai.
The name in the birth certificate obtained then, is different from the school records.
I was advised that the name should be changed through Gazette Notification.
I am presently residing in Bangalore.
Can anyone please guide as to Name change Affidavit, Newspaper Publication has to be given in Chennai or it can be done in Bangalore where I am presently residing.
Also request for a format of the Affidavit.

You can get the format from the Government press, Chennai and publish it at the Tamilnadu Government Gazatte.Thereafter you can attach this notification along with each application.No change in certificate is required.

Dear Sir,
Which local lawyer I have to contact - Bangalore or Chennai? Address in DL and ration card is of Bangalore but the Birth certificate issued by Chennai.

You can get the change of name published in the Government gazette publication of Karnataka government itself. You dont need to even take the help of an advocate, you can visit the government press functioning in Kanakpura Road, get the details for the procedures to be followed in that respect, and get the subject issue done by yourself with very less cost.
