Query on Birth Certificate - Urgent

Respected Experts,

My brother's wife delivered a baby at home 3 years back.Now the child needs to be admitted to school in Mumbai.
But the child's birth records are not available as the child was not born in a hospital but at home in Uttar Pradesh.

Would an affidavit by the father/guardian for the same be enough for admission to the school ?

Please advice as to how I should obtain the Birth certificate ?

1- You have to approach the Local Registrar of Births & Deaths and apply for birth Certificate of Boy - the Registrar Will issue a memo stating that the Birth of the Boy is not recorded -

2- With that Memo file an application & Affidavit before the Reveneu Divisional Officer ( in AP ) for a direction to the Registrar of Births to enter the date of birth of the Boy , on orders of RDO the Reigstrar will enter the event of birth of Boy and Issue Birth certiifcate.

As there is delay in getting the Birth Registered with Registrar , the order for condinging the delay and for entering the Birth envent is required.

These all are formalities in AP - please chek up with Your local Authorities !

In AP- Revenue Divisional Officer ,but you have to check up in your local area who is the Authority .

Every Birth has to be reported and registered within 21 days at the place of its occurrence. After one year, births can be registered by the order of a first class magistrate only.

Eligibility
When the Birth has taken place in a house -the head of the house or the nearest relative of the head of the house, or the oldest person in the family.
When the Birth has taken place outside the house:
(i) In a Hospital/Health Centre, Maternity Home or other similar institutions - Medical Officer In-charge or any officer authorised by him/her

(ii) In a Jail - Jail In-charge

(iii) In a Hospital, Dharamshala, Boarding House, etc. - Person In-charge

(iv) In a Moving Vehicle - Person In-charge of the Vehicle

(v) Found deserted in a public place -
Headman of the Village/In-charge of the local police station


Procedure
A Birth Certificate can be obtained by applying through the prescribed proforma with fee as fixed by local body authorities. The certificates are issued from the registration centres/offices of the local bodies in the zone under which the parents of the child were living at the time of birth.

Proof of Birth of the person for whom the certificate is required.
Affidavit, if birth is being registered after one year of its occurrence.
