Where to register adoption deed?

Divorce done by court order on 1.08.16
Court order to give custody to mother.
Mother is now wanted to do second marriage before that she want to register adoption deed of her child.
Where to register such adoption deed?
Whether NOC of first husband is required?
Expert please advice.

Mother is natural guardian of the child, no reason to adopt own child.
