Sirname missing from 10th certificate

In all my academic documents and education certificates and passport, only my first name is mentioned. i.e. Instead of "Abhishek Srivastava", only "Abhishek" is listed.

Due to this I am facing lots of problem since almost everywhere Sirname is also required. The root cause of the problem is my 10th standard certificate wherein only my first name is mentioned.

Please guide me what to do as I am clueless as what to do. Can I change my 10th standard certificate name.

You can insert your sir-name in your relevant educational certificates by making a representations to the issuing authorities with offer to pay the requisite fee. Failing which, you serve them legal notice followed by civil suit.

such corrections will be done by the issuing authorities,provided,it was a mistake on their part,inspite of correct records like birth certificates etc.

however,if the mistake was on your/parents part,you will have to do the necessary process of name change through gazette notifications.

Even if You miss the all of your Name we have Mr.Shah with us to bring back all the letters as per your desire.

Thanks all for your wonderful and timely response. Just have few questions which I must resolve to make any step further:
1. In my 10th certificate, name is "Abhishek"
2. In my passport, name is "Abhishek"
3. In my PAN card, name is "Abhishek"
4. In my degree certificate, name is "Abhishek"

I want that all the above mentioned certificates should read "Abhishek Srivastava" and not only "Abhishek".

Please correct me if I am wrong. You all are advising to change my name through the process of gazette notifications. But I guess it is only a notification or affidavit which is kind of oath taken by me about my name change. My original documents will still mention my old name. i.e It won't mention my Sirname.

That is why, I had advised you to insert 'Srivastva' in all basic education certificate (SSC) through board of school education from where you obtained that certificate. It is very easy process. Once you get it, all other authorities can be requested to make change accordingly.

You have to file a Affidavit that you used all these names, and you will be known as Mr.
"Abhishek Srivastava",now onward,

All such diff names are of mine, My dateof Birth is xx yy zz, born at of place xxx, mother's name "______" & father's name "______', and you indemnify all concerned for any loss or damage that incur to them for using my name as "Abhishek Srivastava" now onward.With your
photograph.
Also publish in Government Gazette without fail .

No School authority will change your name at your request or otherwise. No Chance.

In your case, I suggest to publish the same in Two News Papers, one local with verna language, and one in English

Thank all of you for your inputs...This means wherever and whenever I need to submit my Ids or address proofs, I'll have to submit the affidavit of name change?

Want to highlight one incident that I faced couple of days before. I applied at ICICI bank for savings bank account. I furnished all the details but wanted to have my name as "Abhishek Srivastava". I asked them if affidavit will serve the purpose but they simply declined to accept any affidavit whatsoever and in the end I have to open the account with "Abhishek".

I simply had no clue why they did not accept the affidavit. If I'll be facing these kinds of problems everytime I require something in the name of "Abhishek Srivastava", then what is the use of the affidavit?
