What are the process for making affidavit of name change for women after marriage?

There is no necessity for change of name for women after marriage. This is just a hype. It is not a law or rule or culture or a necessity for a women to change her name compulsorily after her marriage. She can continue with her old name itself, however since you desire it for some other purpose, you may consult a local lawyer and get the needful done.
