How to leave maiden name of wife?

Name of my Wife is Poonam Khandelwal in 10th class marksheet. After marry, now she is in interest to join business hence need PAN card. But, I am confused how to leave her Maiden Surname 'Khandelwal'.
Is there need of affidavit for same or its Natural thing that a women leave her maiden surname & adopted husband surname?

Get the marriage certificate from local authorities and it should resolve all such matters.

For PAN CARD DOB proof is not mandatory.

Out of various ID proofs you can attach any one.

Name of father in place of husband is mentioned.

Yes, from marriage certificate the surname of wife gets changed.

PAN card contains the name of father.
You can enter the name of your wife with your surname after marriage.
Submit the Id. proofs.

After marriage the surname automatically changes, so obtaining a marriage certificate with the new surname will solve the issue.
