DEAR SIR/MADAM, I WAS WORKING WITH A PVT. LTD. FIRM, & IN THE EPF A/C MY NAME'S SPELLING IS WRONG. I RESIGN/QUIT MY JOB 03 MONTH AGO. NOW I WANT TO WITH DRAW MY EPF. WHAT I SHOULD TO DO
MY CORRECT NAME : IQRAM
MY WRONG NAME IN EPF A/C IQRANI

an affidavit duly supported with documentary evidence that you are the same person should be furnished to the EPF regional office who after being fully satisfied shall effect the necessary changes in their record pertaining to your account.
