The complainant submits as under-

Narrate the facts, with sufficient clarity,which constitute the offences. You must give date, time and place of the incident. Also give names of witnesses if any. divide the complaint in suitable paragraphs.

Also mention in a separate paragraph that the incident has taken within the jurisdiction of the court. Also mention the name of the police station.

In last paragraph request the court to send the complaint to concerned police stn u/s 156[3] of Cr. P.C.
Put date and signature with name.
