Procedure of change of pan

(Querist) 04 July 2013 This query is : Resolved 
If two companies are merged in the matter of Composite Scheme of Amalgamation approved by Court of Law and Transferee company has got changed the name after merger, How to change its PAN no. whether New PAN to be applied OR Transferee company apply for name change?

is any prescribed format for changge, what documents need to to enclosed , to whome we should approach
