Change of name

Sir, I and my wife (hindu while marrying)fell in love and married against both of parents wish 15 years ago. Since then she is known with a new name. Now we need to change her name officially, in all academic and civil records. is it possible? and what are the procedure. please advice.

She can start using new name but can not change what has already been mentioned in the educational records.

After marriage, a woman can start using new name using part of her husband's name.
