How to file a case against an insurance company for claim being rejected without any justified ground?

Preferably under expert advise of your counsel specializing in consumer matters.

"without any justified ground" means? Whether the rejection is as per the terms of the medical policy or not is to be verified not your considered opinion. If the rejection is against the terms, go to consumer court.
