Addition of surname in birth certificate for own and father/mother also

Hi All,

Can somebody help me with the problem I am facing.

I want to apply for a fresh passport. My date of birth is 25th Feb,1991.

Issue is my surname,my father/mother's surname is missing from my birth certificate where as exist in all other certificate.

Please suggest what is the solution for this problem??

Get an affidavit done through a Magistrate.

contact passport authority for required documention.

This is minor mistake, can show it as name change in the passport application or get new birth certificate issued from Municipal Authorities after rectification of the name.
