In all my documents my name is karthik. for applying passport is surname compulsory. if so how to add surname.

You give your name what is available in your documents, do not indulge in any smart activity by adding any other name just to add surname.
If there is no surname attached to your name then there is no necessity for you to add surname to your name as suffix.

From the beginning of your studies if you/your parents did not mention your surname anywhere now there is no necessity to add a surname. You may consult the passport authorities.

You cannot add surname for the purpose of issuance of passport till it is published in gazzete.
