Process to get duplicate fd

Dear Sir,
My SBI FD has been lost and maturity date was 28 Nov 2012. FD was done thru sbi account only.Now how can i get duplicate FD.

duplicate FD.

you will have to make an application for issue of duplicate fixed deposit receipt .
bnak will call upon you to execute indemnity bond in prescribed form with one or more sureties or guarantee
