want to add surname

Dear sir,
my name is Manish in all my documents(school certificate,DOB certificate etc.) but i want to add a surname(Mathur) with my name.My Father's name also doesn't include any surname.
Please tell me how can i change it in all my documents?

Make a affidavit before a first class magistrate to that effect and get it published in the news paper.

It amounts to correction is the documents, so first you have to issue notice to the Concerned State Govt., rep., by the Deputy commissioner and Deputy Director of Public instructions u/s 80 of CPC and after completion of mandatory period of 60 days you will have to file a suit for delcaration and injunction.
A notice should be just like a plaint.
