No standard format. Application fee can be paid along with application either at the cash counter of the organisation or by attaching a postal order of Rs.10.

No standard format.

The prescribed fee can be paid through postal order / Bank draft or cash on counter if facility available.

I too agree with the experts opinion that there is no specific format for this, one can write it in a letter format too.
