I can give you the format of rental agreement. But before that you have to give me the following details (1) who are you tenant or owner (2)what is the period of lease (3) What is the rent (4)what is the security deposit (5)for what purpose the premises should be used (6)what is the notice period (7) complete details of the premises with boundaries etc.

Rent agreement require notary or any other thing similar to notary?

If rent agreement is for more than 11 months, it requires compulsory registration.

Rental agreement do not require notary. Lessor, lessee and two witnesses is enough and it should be registered. In Andhra Pradesh the Act has been amended. Now even for one month also registration is compulsory. I do not know the legal position of other states.
