Procedure for obtaining disability certificate

I met with an accident at Jind with a motor cycle ITFAQIA and a DDR was registered in this regard,later on i filed a claim petition us 163 A MV Act,now claim case is pending my evidence, i was operated for fracture in my leg.I want disability certificate ,kindly tell me procedure for the same.Thanx

get a medical certificate from ortho doctor to whom you have taken treatment. another way give the discharge summary to another ortho doctor who has to give evidence based on your body after examination and give disability certificate. Contact your lawyer and a local doctor(ortho)

Move an application to MACT wherein your claim petition is pending or to local SDM Jind to direct Civil Hospital Jind to assess your disability and issue certificate accordingly.

The certificate will be issued by the Civil Hospital Surgeon after examining the fracture and disability he decides the percentage on the basis of his examination.

for this move an applilcation to the Board of Doctors for issuing the disablity certificate in the civil Hospital and they will examine your physically and then issue the certificate to you.

You need to apply at C.M.O. office of your district where a medical board sits every weak for the purpose,the application is moved in a prescribed format available there ,for which you need to carry proof of your residence,voter id card,etc and also your photograph in duplicate.

The same day board of doctors will examine you and you shall there after be given the certificate.
