Name change certificate

Hello Dear.

I have changed my name twice officially and was published in official gazette duly all the two times. my question is, if, there is any certificate or form declaring or showing change of name twice in same certificate or same card.

There is no such certificate or form to deal with such situation. The latter shall prevail over the previous notification.
