seniority loss joining date delay

(Querist) 18 November 2015 This query is : Resolved
Hi,
I'm expecting to join as Assistant in ministry of external affairs. I haven't got the joining letter yet. I am waiting for 4 months and I don't know when the formalities would complete.
On the other hand, I have a surgery pending. The recovery time for that would be around 4 months.
My questions are:
1. If I go ahead with surgery now, since I don't know when the joining date will be, would it hamper my prospects in promotion due to loss of seniority assuming I get joining before recovery and I have to ask for extension in joining date?
2. If it's an option to join with medical certificate of doctor and ask for extraordinary leave on the very first date of joining?

Thanks in advance for your help.

The seniority is determined by order of merit in select list.
