Add surname/last name to my name

Dear Sir/Madam,
Always Thanks for LCI contributions.
My name is Brijesh Kumar on educational certificates ie,10th class and Graduate.The same name is also on Voting card/DL/PAN.But on professional qualification certificate it is Brijesh Kumar Goyal as I added my surname"Goyal" being my sub caste(also being inhabitant of using Goyal)unintentionally on only last exam.form application.
As now I get aware of and am precaution for future,
So Pls.Provide me legal advise:
-Should I get correction in professional certificate or not ?(Note Year of qualification-2006)
OR)
Can I make an affidavit for two names being related to one and same person,if it will work in future,if needed.
If I want "Goyal" to be part of my name in future:
-Should I get "Goyal"added to Voting card/DL/PAN etc? If yes what is the way out.
-Can I make an affidavit for name change/adding "Goyal" and publish in local news paper of my address.
-Is publication in gazette is mandatory as it is just mere addition of last name.
Pls.Guide me.

It will be better to keep the documents with a single name.
Every state has its own rules in this respect, better visit some lawyer or an assistant working on Administrative side to get the formalities done as per procedure adopted by the concerned state govt.

you should have same name Brijesh Kumar as reflected in your X std school leaving certificate and also voter cards, PAN card .
get name corrected in your professional certificate

It would be better to keep your name as is on Xth standard certificate and other documents, in such case you have to complete less formalities in changing the name in documents.
If you want to keep Goyal with your name in future, you can go for affidavit, news paper + gazette publication and than get the name changed on all other documents.

Properly advised by experts, I agree with them. If the surname to be suffixed to your name, a gazette publication to that effect will provide you the solution.
