More than one names of my wife

After marriage in some documents my wife uses the name given from my house. Her title got changed after marriage. What affidavit is suitable for this.

School Name X Y

After marriage Z Mandal and X Mandal.

Your wife can make an Affidavit on oath before a Magistrate declaring her name in the school and her name after her marriage are the same and one identical person. After that said declaration ought to be published in two local news papers where she lives.
