For making a PAN application, there is Affidavit sworn before a magistrate stating the date of birth is acceptable as proof of date of birth.
Kindly clear below :
(1) if this affidavit must be form on a stamp paper ?
(2) is there any format provided by income tax department ?
(3) if this affidavit should be notarized by a notary public ?

(1) The affidavit should be on a stamp paper.
(2) There is prescribed format for it.
(3) Consult IT department or implementing agency.
