Conversion from hindi to muslim

I want to become muslim but I am not getting shahada certificate, as the maulvi is saying they will perform the rituals but they don't have certificate what to do

For conversion to Islam, one must visit a mosque in the locality and take Shahada in the presence of a Maulvi and two major witnesses. Once the Shahada is performed, the Maulvi will issue a conversion certificate on the mosque's letterhead, called a Shahada certificate. Consult the head of the Mosque of your locality.
