Spouse surname change process

I want to add my spouse's name in my passport.But I want to add the name with my family name/surname.I have affidavit for the same.Will that be sufficient or I have to manage any other document.I have marriage certificate with maiden name of my spouse.Please assist.Thanks!

@Author, you have to give an Affidavit stating that before marriage, the name of your wife is ---------- and after marriage she is called as ---------- and it should be got attested by a Notary. in the same manner, your wife has to give an Affidavit stating that before marriage her name is ------ and after marriage she is being called as---- and it should also be got attested by a Notary. and apply with marriage certificate.

Submit the copy of marriage certificate with affidavit.
