Whether there is prescribed form of talaknama ?
I have heard about form prepared by muslim board.
From where i can get it ?
what contenrs are necessary in it ?

There is no specific format for talaknama. U have to mention the grounds of talak while giving a talaknama. If u want to get it from the court then u will have to file Suit for dissolution of Marriage if in case of Mohd.,
