regarding surname

Hello sir!!
My name in school documents is "Kiran Kumari" and in college documents it is only "Kiran"
I have got the affidavit of same person but I have no idea about the next procedure. Please guide me.

Approach the college authorities to change your name accordingly and also the change be effected in university.
